Position: FT RN Case Manager - 4 10's (2 Days On Weekend) OMC Jeff Hwy - In-person - Sign On Bonus

Job Overview

This role handles multidisciplinary communications and facilitation of care throughout a patient’s hospitalization, includes reviewing appropriate use of resources, balancing patient right to self‑determination, recommending level of care and documenting in record. Acts as liaison with patient, caregiver, multidisciplinary team, post‑acute and third‑party payers. Collaborates with care team to develop discharge plans based on individualized needs.

Responsibilities
  • Review pre‑admission, admission, and continued stay as directed per CMS guidelines and hospital policy.
  • Recommend medically necessary hospital care level and document clear level of care order.
  • Communicate with ED physicians, hospitalists, attending physicians, and physician advisors to coordinate discharge planning.
  • Develop and implement discharge plans and follow‑up as needed.
  • Maintain open communication with all appropriate parties during acute‑care hospitalization.
  • Adapt communication style to patient population, respecting privacy and obtaining permissions.
  • Perform other related duties as required.
Qualifications
  • Graduate of an accredited school of nursing.
  • Bachelor’s degree in nursing preferred.
  • No work experience required; preferred 3 years hospital‑based experience in discharge planning, case management or utilization review.
  • Current registered nurse license in state of practice required.
  • BLS certification from American Heart Association required; case management certification (CCM or ACM) preferred.
Knowledge, Skills & Abilities
  • Proficiency in using computers, software, and web‑based applications.
  • Effective verbal and written communication skills; ability to present information clearly and professionally.
  • Strong interpersonal skills.
  • Ability to work a flexible schedule (24/7, weekend, holiday, on call).
Physical & Environmental Demands

Medium work involving lifting 20‑50 pounds occasionally or 10‑25 pounds frequently; requires walking or standing significantly. Exposure to blood, body fluids and potential communicable diseases. Potential exposure to hazardous medications and waste.
