WRAPAROUND FACILITATOR

Summary

The Wraparound Facilitator serves as the primary contact person for the families and leads with a strength-based, family-centered, culturally competent approach to meet the complex needs of the youth. The Facilitator works in partnership with team members to develop an individualized plan of care, oversees the implementation of the plan of care, identify providers of service and/or resources, facilitates monthly child and family team meetings, conducts regular home visits, and monitors all authorized services.

The Wraparound Facilitator reports to the Wraparound Supervisor.

Essential Functions
  • Facilitate a high-fidelity wraparound process for youth and families, providing individualized, coordinated, family-driven care to meet the complex needs of the youth.
  • Coordinate care with formal behavioral health providers, waiver service providers, Primary Care Physician (PCP), Family Service Organization (FSO), and other relevant stakeholders at critical points of enrollment.
  • Facilitate and coordinate a process that requires the youth, family, providers, and natural supports to collaborate, building a strong support system for the youth and family, as well as building a Plan of Care that addresses the needs of the youth and meets the requirements of National Wraparound Initiative (NWI) standards.
  • Build trusting, professional relationships with the youth, families, and providers.
  • Provide support to the families in identifying strengths, needs, cultural values, and priorities.
  • Identify and coordinate services across multiple systems (behavioral health, education, child welfare, juvenile justice, etc.).
  • Track progress towards goals, monitor service delivery, and adjust the Plan of Care as needed.
  • Ensure services are accessible, culturally appropriate, and outcome focused.
  • Maintain accurate and timely records of meetings, plans, and progress notes in accordance with the state, VOASCLA, and funding requirements.
  • Build and regularly update the individualized Plan of Care based on the family’s goals and strengths.
  • Partner with schools, agencies, community organizations, and natural supports to strengthen the family’s network.
  • Collaborate with the Wraparound Clinical Director and/or assessors to ensure completion of assessments and all recommendations are incorporated into the Plan of Care.
  • Encouraging the youth and family to celebrate “wins” where progress has been made.
  • Guide the Child and Family Team Meetings (CFT) in evaluating what is working and what needs to be adjusted to ensure youth and family success.
  • Help to maintain CFT cohesiveness and satisfaction, supporting fidelity to Wraparound principles; managing any disagreements, conflicts, or dissatisfactions that may arise.
  • Convening and facilitating CFT meetings monthly at a minimum, and more frequently if necessary.
  • Assisting the family in identifying team members.
  • Maintain confidentiality as it pertains to all aspects of clients’ care, staff, and VOASCLA.

Required

Education and Experience
  • Bachelor’s degree in a human services field. Bachelor’s degree in any other field requires a minimum of two (2+) years of full-time experience working in relevant family, children/youth, or community service capacity.
  • Relevant alternative experience may substitute for the Bachelor’s degree but is subject to the approval of LDH.
Other Requirements
  • Maintain a valid Louisiana driver’s license
