Community Support Specialist; Lafayette SSVF

Overview

We are seeking a Community Support Specialist for our Lafayette location. In this role, you will work closely with Veterans and their families, providing supportive services and resources to help them obtain and sustain permanent housing and achieve greater stability within the community.

• Provides case management and supportive services.
• Participates in setting case plan goals/objectives.
• Completes case logs and required documentation on all client services, including data entry into required data systems.
• Maintains professional boundaries and operates within agency/ethical guidelines.
• Maintains professional and ethical standards of conduct at all times.
• Promotes recovery, independence, and personal choice for participants.
• Provides resources and referrals to agency and community resources.
• Assists clients in identifying strengths, needs, and realistic goals to develop a service plan.
• Accurately identifies risk and responds appropriately.
• Effectively teaches social skills and household management skills.
• Demonstrates knowledge of behavioral health symptoms, interventions, and impact.
• Knowledge of agency and community resources and referral processes.

• Bachelors degree
• High School Diploma required
• Driver’s License
• Experience working with veterans preferred
