JOB DESCRIPTION
Registered Physical Therapist (PT)
JOB SUMMARY:
A Registered Physical Therapist (PT) administers physical therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the supervision of the Director of Clinical Services/Nursing Supervisor.
QUALIFICATIONS:
Graduate from an accredited physical therapist curriculum (American Physical Therapy Association or equivalent).Currently licensed in the practicing state(s).Two (2) years’ experience preferred.Alignment with the agency's philosophy and goals.Ability to exercise initiative and independent judgment.RESPONSIBILITIES:
Adhere to established policies and procedures.Provide physician-prescribed physical therapy.Help improve or minimize residual physical disabilities.Assist patients in returning to their optimal and productive levels.Participate in patient care planning with home care team.Supervise personnel as required.Conduct initial history taking and evaluation.Perform all skilled procedures as ordered.Consult with physicians regarding treatment changes.Report patient progress to physicians.Instruct patients and families in home programs and daily activities.Participate in in-service training programs.Engage in Quality Assessment and Performance Improvement activities.Attend scheduled patient care conferences.Prepare clinical and progress notes.Assist in evaluating patient function levels.Develop and revise the plan of care as needed.Consult with family and agency personnel.Complete and submit OASIS assessments and other required documentation timely.Utilize ICD-10 codes appropriately.WORKING ENVIRONMENT:
Indoor work at agency office and patient homes, with travel involved.

LIFTING REQUIREMENTS:
- Participate in physical activities as needed.
- Work extended periods standing and involved in physical activity.
- Heavy lifting may be required.
- Perform extensive bending, lifting, and standing regularly.
