Wellstar Health System in Georgia seeks a Home Health Physical Therapist to provide and supervise primary care for 25–35 patients in their homes. Coordinate with RNs, therapists, social workers, physicians and families to ensure an effective plan of care and timely visits.
The role requires PT education, state licensure, BLS, and 1 year of PT experience; home health preferred. You will supervise PTAs and Home Health Aides, participate in quality improvement, and maintain CMS/OASIS documentation.
