Occupational Therapist

Role

Occupational Therapist specializing in home health care, providing age-appropriate interventions to meet patients' physical, emotional, developmental, psychosocial, and educational needs.

• Assess and manage patients' rehabilitative needs, developing and revising individualized care plans in collaboration with the care team.
• Deliver high-quality therapy through assessment, therapeutic exercises, education, and treatments in patients' homes.
• Coordinate services with clinical supervisors and team members, ensuring communication with physicians regarding patient progress.
• Educate patients and caregivers to promote independence and support recovery, using verbal and written tools.
• Participate in staff training, continuing education, and in-service activities to maintain clinical competence.
• Ensure accurate documentation of interventions, patient progress, and regulatory compliance within designated time frames.

• Graduate of an accredited Occupational Therapy program.
• Licensed as an Occupational Therapist with BLS certification.
• Minimum one year of full-time occupational therapy experience; home health experience preferred.
• Strong assessment, rehabilitative, and critical care skills.
• Excellent problem-solving, organizational, and communication skills.
• Proficiency with computer software, electronic medical records, and understanding of home health regulations.
