Occupational Therapist

Role Overview:

The Occupational Therapist provides age-appropriate rehabilitative care, addressing physical, emotional, developmental, psychosocial, and educational needs of patients in a home health setting. They collaborate with multidisciplinary teams to develop and revise personalized care plans, ensuring all goals are met.

• Deliver direct patient care through assessments, therapeutic exercises, education, and treatments, managing patient needs from admission to discharge.
• Coordinate services by developing and adjusting care plans in accordance with physician orders and patient progress.
• Educate patients and caregivers to promote independence and support recovery.
• Participate in staff training, continuing education, and professional development to stay current with clinical practices.
• Accurately document interventions, communicate patient status changes to physicians, and adhere to regulatory guidelines.

• Graduate of an accredited Occupational Therapy program with a valid license and certifications, including BLS and driver’s license.
• Minimum of one year of full-time occupational therapy experience, preferably in home health.
• Strong assessment, rehabilitative, organizational, and communication skills; proficiency with computer systems and home health operations.
