Physical Therapist – Home Healthcare

Role Overview:
The Physical Therapist provides primary rehabilitative care in patients’ homes, managing 25-35 individuals, and supervises PTAs and Home Health Aides. They develop and coordinate personalized care plans, ensuring goals are met through collaboration with multidisciplinary teams including physicians, nurses, social workers, and caregivers.

• Assess patient needs and perform routine and complex therapies based on physician orders.
• Coordinate services throughout the episode of care, adjusting care plans as needed.
• Supervise and review care provided by support staff, ensuring quality and compliance.
• Educate patients and caregivers to promote independence and self-care.
• Communicate patient status changes to physicians and participate in team conferences.
• Ensure documentation complies with regulatory standards, including CMS and Joint Commission guidelines.
• Participate in performance improvement initiatives and ongoing professional development.
• Meet productivity standards and perform additional duties as assigned.

• Graduate of an accredited physical therapy program.
• Valid Physical Therapist license and BLS certification.
• Georgia Driver’s License and private vehicle insurance.
• Minimum of 1 year full-time physical therapy experience; home health experience preferred.

Skills:

Strong assessment, rehabilitation, leadership, communication, organization, and time management skills. Proficiency with Windows, home health software, and understanding of regulatory processes is essential. OASIS knowledge is preferred.
