Job SummaryCore Responsibilities and
Essential Functions:
Exemplary Practice and Outcomes:
Performs the Nursing Process (assessment; diagnosis; identification of outcomes; planning; implementation and evaluation) in clinical care using evidence‑based practice, analytical and critical thinking, ensuring individualized care. Coordinates care through effective partnerships recognizing that caring is central to optimal patient outcomes. Provides relationship‑based, patient‑centered care consistent with population‑specific characteristics (age, gender, disease, etc.) and adapts service delivery to reflect cultural diversity.
Partners with patient and significant others using appropriate methods (bedside shift report, interdisciplinary rounds, just‑in‑time and planned patient teaching) to keep patients and their families updated and make patient goals the focus of the plan of care. Practices using current clinical practice standards.
Coordinates delivery and documentation of safe, quality patient care that promotes the professional care delivery model, including interdisciplinary communication methods such as rounds and case reviews; completes timely documentation, promotes a respectful, inclusive clinical environment. Demonstrates teamwork by practicing in a fiscally responsible manner that supports recruitment and retention of all team members. Supports performance improvement and shared governance at all levels.
ProfessionalDevelopment and Initiative
Completes all initial and ongoing professional competency assessments and mandatory education; serves as a preceptor/mentor for other professional nurses and staff to ensure a qualified workforce and a healthy work environment.
Evidence Based Practice and ResearchPromotes evidence‑based practice and nursing research, participates in activities such as value analysis, ensures adherence to policies and professional standards, collects data, poses relevant clinical questions, consults experts, and applies evidence to practice.
Resources and SupportPlans for patient care across the continuum, develops protocols and procedures with the healthcare team, supports efficient use of human and material resources, and performs other duties as assigned. Complies with all Well Star Health System policies.
RequiredMinimum Education
- Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing (Preferred)
- RN – Reg Nurse (Single State) or RN‑COMPACT – RN – Multi‑state Compact
- BLS – Basic Life Support or ARC‑BLS – American Red Cross Basic Life Support or BLS‑I – Basic Life Support Instructor
Minimum 1 year recent, acute care clinical nursing experience.
