Nursing Assistant; Care Partner - Med​/Surg - Nights

Work Shift: Night (United States of America)

The Care Partner is a support care provider who functions under the direction of the Registered Nurse. The Care Partner’s primary focus is to promote a positive patient experience by partnering with the nurse, physician, healthcare team, and other healthcare providers to provide excellence in patient experience and assist with daily living activities and comfort measures. The Care Partner assists other health care providers with safe, age‑appropriate and culturally aware care by performing basic patient care functions, such as ADLs, ambulation, and vital signs.

Additional duties may include advanced technical skills such as phlebotomy, simple treatments, dressing changes, oral suctioning, and discontinuing Foley catheters, as well as clerical functions: retrieval of data, maintaining unit supplies, and transporting supplies/patient medications. The role requires working in a fast‑paced environment with multi‑tasking and effective communication, performing under stressful conditions with a compassionate, respectful, and service‑mindset, and following direction from various care providers.

• Provide excellent patient experience and collaborate with the care team, patients, and families.
• Support patient‑centered care: administer basic care needs, including hygiene, nutrition, and physical comfort measures; perform advanced skills such as phlebotomy, EKG, and removal of Foley catheters and IVs when required.
• Assist with patient admission, data entry for key patient safety information, charge reconciliation, and audits.
• Participate in hourly patient rounding and provide timely responses to telephone calls and call lights.
• Resolve patient concerns or notify the charge nurse if escalation is required.
• Offer proactive assistance and comfort measures to patients and families.
• Transport unit supplies, medications, and equipment as assigned.
• Maintain confidentiality and adhere to HIPAA regulations.
• Practice safe handling, use of PPE, and appropriate infection control measures.
• Conduct environment of care rounds and participate in unit performance‑improvement initiatives.
• Communicate effectively with patients, staff, physicians, and families using AIDET and other communication protocols.
• Document patient care activities and communication in the EMR and report issues or emergencies in a timely manner.
• Maintain patient care equipment and report non‑functional devices.

• Support initiatives of the healthcare team.
• Participate in unit performance improvement initiatives.
• Help maintain professional nursing practice through shared leadership, evidence‑based practice, and ethical accountability.
• Practice accountability and uphold the code of conduct.

• Use effective written and oral communication skills with all stakeholders.
• Apply safety communication techniques and chain of command protocols.
• Document and communicate completed assignments and patient care concerns to RN or charge nurse.
• Use positive and respectful communication with patients, families, and healthcare team.

• Provide a safe environment for patient care and consistently follow hand hygiene and PPE policies.
• Keep patient rooms and unit areas clean and free of clutter.
• Reinforce universal precautions with patients and families.
• Use proper body mechanics, patient‑safe handling devices, and assistive devices.
• Perform environment of care rounds and other assigned duties.
• Comply with all Wellstar Health System policies, standards of work, and code of conduct.

• High School Diploma (General‑Preferred)
• GED (General‑Preferred)

• BLS – Basic Life Support
• ARC‑BLS – American Red Cross Basic Life Support
• BLS‑I – Basic Life Support – Instructor

• Graduation from a state‑approved program for Certified Nursing Assistants or Patient Care Tech Program (Preferred upon hire)

• Direct patient care experience (preferred)

• Ability to follow detailed directions

• Ability to work individually and as part of a team
