In-Home Nurse Practitioner or Physician Assistant - Burlington

About the Role

Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

• The visit lasts about 45 minutes and includes medication and medical history review, a physical evaluation, and optional diagnostic tests such as diabetic eye exams and spirometry.
• You will use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.
• The role offers flexible, nationwide travel and virtual visit options as needed.

• Build trusted relationships to help people become healthier.
• Participate in a national network of purpose‑driven clinicians focused on improving health outcomes.
• Provide Signify Health services, including diagnostic and preventive products such as spirometry, DEE, bone density screening, and more.
• Stay flexible to travel locally and within licensed states.
• Conduct virtual visits when required.

Your primary workplace will be the private residence of health plan members. You must be able to perform your duties safely in homes that may contain pets, secondhand smoke, or other allergens.
