Palliative Care Nurse, Nurse Practitioner
Position Summary
As a Per Diem Hospice Nurse Practitioner, you will provide consultation in palliative care, symptom management, and supportive care to meet the needs of hospice patients as requested by the referring physician.
Core Responsibilities- Make face‑to‑face encounter visits within prescribed time limits to qualifying hospice patients and report clinical findings to the hospice medical director.
- Consult with the hospice medical director as needed, inform the primary physician of recommended services, and collaborate with other physicians as required.
- Initiate and coordinate the plan of care, documenting problems, appropriate goals, interventions, and patient/family response to hospice care.
- Collaborate with the patient/family, attending physician, and other interdisciplinary team members to provide patient and family care.
- Instruct and supervise the patient/family in self‑care techniques when appropriate.
- Maintain accurate and relevant clinical notes regarding the patient’s condition and report patient condition and any identified needs/issues to the administrator and hospice medical director.
- Active unrestricted license in the state of residence.
- Current certification as a clinical nurse specialist or nurse practitioner in the state of practice.
- Current certification by a national body such as ANCC, NBCHPN, or AANP.
- Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation.
- 5+ years of nursing experience.
- Current CPR certification or ability to complete it within 90 days of hire.
- Clinical nursing experience in oncology, hospice, geriatrics, or intensive care.
- Palliative Care ARNP certification.
- Experience working with an interdisciplinary team.
Pay is based on several factors, including local labor markets, education, work experience, certifications, etc. The hourly pay for this role will range from $46.00 to $69.00 per hour based on full‑time employment.
