Home Health Licensed Practical Nurse

• Provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN
• Implement individualized care plans with patients, families, and the healthcare team
• Educate patients and families on disease management, treatment options, and self-care
• Document patient visits, interventions, outcomes, and other care records accurately
• Communicate promptly with patients, physicians, clinical managers, and healthcare team members about needs and status changes
• Participate in care coordination and discharge planning

• Current nursing license in the practicing state
• Valid driver's license, auto insurance, and reliable transportation
• Current CPR certification
• Two years' experience as an LPN/LVN in a clinical setting
• Nursing experience in a Home Health or Hospice setting (preferred)
• Required TB screening for this patient-facing role
• Maintain personal vehicle liability insurance in accordance with residing-state requirements or specified minimum limits

Demonstrates expertise in providing personalized nursing care, implementing individualized care plans, and educating patients and families on disease management. Proficient in documentation and communication within a healthcare team to ensure effective care coordination.

• Current Nursing License
