$70K–$75K Base + Commission/Incentive

Home Health Sales Experience Required

Are you an experienced Home Health sales professional who knows how to build referral relationships, grow census, and turn connections into admissions?

A growing, established Home Health organization is seeking a driven Patient Care Representative / Navigator to expand referral relationships throughout the Greater Lake Charles, LA area including East to Opelousas and North to Kinder, LA.

• Develop and maintain strong referral relationships throughout your assigned territory
• Build relationships with physicians, discharge planners, case managers, hospitals, SNFs, assisted living communities, rehab facilities, and senior living organizations
• Generate qualified Home Health referrals and help convert referrals into admissions
• Grow patient census through strategic business development
• Conduct daily field visits with existing and prospective referral sources
• Cold call and develop new accounts
• Create and execute an effective territory sales plan
• Conduct presentations for physicians, nurses, healthcare leaders, and community organizations
• Track referrals, admissions, account activity, and sales performance
• Partner closely with intake, clinical leadership, and operations to ensure a smooth referral-to-admission process

2+ years of Home Health sales, PCR, liaison, navigator, account executive, or healthcare business-development experience

Proven success generating Home Health referrals and admissions

Established relationships with physicians, hospitals, discharge planners, case managers, SNFs, ALFs, or other healthcare referral sources

Ability to cold call, prospect, and open new accounts

Documented sales success, census growth, awards, rankings, or quota achievement preferred

Strong knowledge of the Houston healthcare market

Excellent presentation, networking, and relationship-building skills

Self-motivated and comfortable managing an independent field territory

Valid driver's license and reliable transportation

$70,000–$75,000 base salary
