Position: MDS Director (RN/LPN)

Facility: Lake City Healthcare and Rehabilitation

Location: Lake City, Florida

Status: Full-Time

Compensation: $37.00-$40.00 per hour (based on experience)

Lake City Healthcare and Rehabilitation is seeking an experienced and highly organized MDS Director (RN/LPN) to lead and oversee the MDS process while ensuring regulatory compliance and high-quality, resident-centered care. This key leadership role supports accurate care planning, clinical excellence, and strong interdisciplinary collaboration.

• Oversee the accuracy, completeness, and timeliness of all MDS assessments and documentation
• Ensure compliance with state and federal regulations, including Medicare and PDPM requirements
• Conduct comprehensive resident assessments and interviews
• Develop, implement, and monitor individualized care plans for short- and long-term residents
• Lead and participate in interdisciplinary team meetings
• Collaborate with nursing staff, caregivers, physicians, specialists, and families to optimize resident outcomes
• Provide leadership, education, and ongoing support to nursing staff
• Conduct in-service training related to MDS, RAPS, Triggers, PDPM, and Medicare guidelines
• Maintain MDS and care plan schedules for both Medicare and non-Medicare residents

• License:
  Active Florida RN or LPN license in good standing
• Experience:
  
  Minimum of 2 years of MDS Coordinator experience in a Skilled Nursing Facility
• Knowledge:
  Strong working knowledge of MDS, PDPM, Medicare guidelines, and care‑planning processes
