Role Overview
The Center Well Home Health LPN reports to the Clinical Manager and provides one‑on‑one care to patients in their homes, helping them regain independence and support their recovery journeys.
Responsibilities- Provide skilled nursing care to patients onsite, under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families on disease management, treatment options, and self‑care techniques.
- Maintain accurate records of patient care, including documentation of visits, interventions, and outcomes.
- Maintain timely contact with patients, physicians, the clinical manager(s), and other healthcare team members regarding patient or family needs, status changes, care coordination, and discharge planning.
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting.
- Experience in Home Health or Hospice settings.
Pay Per Visit/Unit Rate
: $49.00 – $69.00
Scheduled Weekly Hours
: 1 hour
Annual Pay Range
: $53,000 – $73,000 per year
Limited‑term, variable‑schedule, and per‑diem associates are eligible for benefits designed to support well‑being, including paid time off, a 401(k) retirement plan, employee assistance programs, and coverage for business travel and accidents.

:
This role is patient‑facing and part of Humana’s Tuberculosis screening program. Selected candidates will be screened for TB.
Driving Statement
:
The role requires a valid state driver’s license and maintaining personal vehicle liability insurance with limits of $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 property damage, or higher if required by state law.
