This position is subject to a level 2 background screening. For more information on the Florida state background screening process and standards, review the Care Provider Background Screening Clearinghouse.

Responsibilities
  • Provide medical care, palliative treatment and manage terminal illness in compliance with the state Nurse Practice Act under supervision of a collaborating physician.
  • Complete history and physical examinations by evaluating patient medical condition and health history.
  • Complete hospice face‑to‑face visit requirements prior to the third and subsequent hospice benefit periods to assess eligibility.
  • Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification.
  • Maintain accurate clinical notes.
  • Communicate with physician and participate in interdisciplinary meetings to meet medical and emotional needs of patient, family, and caregivers.
  • Ensure compliance with all state and federal legal and regulatory requirements.
  • Perform additional responsibilities as assigned.
Qualifications
  • Graduate from an accredited School of Nursing and an accredited Nurse Practitioner program.
  • Current DEA license (if applicable).
  • Current nursing license in state of practice.
  • Current CPR certification.
  • Master’s degree with a minimum of one year NP experience.
  • One year hospice or palliative care experience preferred.
  • Accreditation from an approved certifying body for advanced practice nursing required by state of practice.
  • Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN) or Geriatrics).
  • Commitment to clinical and documentation excellence.
