Job Summary And Qualifications

POSITION SUMMARY:

Responsible for the identification, clinical evaluation, and tracking of in-house stroke patients. Directs ongoing patient tracking, chart abstracting, and data entry of stroke patients into all stroke databases. Responsible for patient education, participating in multidisciplinary rounding, quality assessment of patient care to include assisting with design and development of PI indicators, active participation in outcome review, and initial screening for peer review issues.

All stroke registry data and PI activities will comply with Joint Commission, state and national guidelines. Provides ongoing statistical reports and assists with developing specialized reports as required. Participates in stroke research as appropriate. Serves as a liaison between HCA Florida Gulf Coast Hospital, outreach facilities, and EMS. Works in collaboration with Stroke Medical Director, Emergency Department, clinical units where stroke patient are cared for, and all other ancillary departments.

Performs other related duties as required or assigned.

QUALIFICATIONS

Registered Nurse with a minimum of 2-4 years of recent experience in their field of specialty. Prior knowledge of stroke treatment and management is preferred. They should demonstrate competency in the four-prong areas of education, clinical/technical skills, research, and outreach.

Educator/Academic & New Hire Liaison
  • Develops and assists with implementation of hospital-wide stroke awareness education.
  • Participates in mentorship of new staff during orientation or cross-training for program population.
  • Designs and conducts formal and informal training to enhance team knowledge and operational efficiency.
  • Educates staff, outreach facilities, and EMS on the Stroke Standards of Care.
Change Agent/Team Member
  • Identifies opportunities for improvement by incorporation of data and patient outcomes into continuous quality improvement process.
  • Collaborates with the multidisciplinary team of healthcare professionals in the development, implementation, monitoring and revision of written care protocols.
  • Designs and implements PI activities, analyzing results to improve both the PI and training initiatives.
Researcher/Consultant
  • Researches and presents pertinent clinical literature to healthcare team.
  • Serves as clinical expert resource to healthcare team.
  • Demonstrates, via role modeling and consultation, advanced knowledge and practice in stroke patient care.
  • Develops and reviews policies and procedures in relation to the care of the stroke patient, consistent with evidence-based practice recommendations.
Leader/Communicator
  • Communicates with core team members, nursing directors, clinical staff and physicians to facilitate identified issue resolution.
  • Manages the interdisciplinary components of the stroke program via effective communication skills.
  • Coordinates with nursing directors throughout the hospital and outreach facilities to ensure standardized, evidence-based care of stroke patients.
Outreach/Patient Education
  • Serves as a patient advocate promoting patient access to services, tools, and medications needed to ensure successful stroke care management.
  • Ensures follow-up care is identified, ordered and scheduled for patients discharged from acute care for hub facility.
  • Coordinates with the hub healthcare team to provide patient/family education regarding post-stroke care and management.
  • Develops relationships and communication protocols with physician groups and community to promote stroke care and management.
Program Specific
  • Directs/completes concurrent and retrospective chart abstracting and data entry for stroke patients into the stroke registry databases.
