Clinical Community Liaison

As a Clinical Community Liaison, you will:
- Meet with hospitals, nursing homes, and community providers to promote awareness of hospice care.
- Help identify patients who may benefit from hospice services and assess their eligibility.
- Coordinate care between referral sources and our hospice admission team.
- Support patients and families through the transition from hospital or home to hospice care.
- Track and understand local healthcare needs, referral trends, and competitors.
- Participate in outreach events and support the marketing team with planning and strategy.

Qualifications:
- Registered Nurse with current licensure to practice in assigned state(s)
- Bachelor's degree in Nursing from an accredited program by the National League for Nursing preferred
- Three years' experience in clinical nursing, hospice experience preferred
- Two years of experience in compliance, internal audit, basic law or paralegal strongly preferred
- Excellent understanding of compliance, Medicare Conditions of Participation, and understanding of medical documentation standards
- Ability to manage highly confidential information
- Analytical skills with ability to define problems, collect data, establish facts, and draw valid conclusions
- Meticulous attention to detail
- Self-director and ability to work with minimal supervision
