Registered Nurse (RN) Case Manager — Part Time

We are seeking a compassionate and dedicated Registered Nurse to serve as a Case Manager in our East Metro home care team, delivering skilled nursing care and overseeing client services in their homes. The role offers 3 days a week with occasional weekend or evening work, supporting a healthy work‑life balance.
Responsibilities- Conduct initial and ongoing assessments of clients to determine their care needs.
- Develop, implement, and update client care plans in collaboration with physicians and the home health team.
- Provide direct skilled nursing care to clients of varying ages and acuity levels.
- Coordinate care with families, caregivers, and other healthcare providers.
- Medication Management:
Ensure proper medication administration and reconciliation. - Client Monitoring:
Regularly monitor clients' conditions and report any changes to the appropriate personnel. - Provide education and support to clients, families, and caregivers.
- Ensure accurate and timely documentation of client care and progress.
- Ensure safety needs of clients are addressed through home environment assessments and individualized abuse prevention plans.
- Provide clinical oversight, delegation, and leadership to other licensed and unlicensed personnel.
- Maintain accurate and up-to-date client records.
- Adhere to all relevant regulations, including OSHA and HIPAA.
- Manage schedule effectively and participate in on‑call phone rotation as needed.
- Licensed and unrestricted Registered Nurse (RN) in the State of Minnesota or Wisconsin.
- Certified in CPR and have a negative TB skin test or chest X‑ray.
- Minimum 1 year of experience in a health care facility or home care setting; home health experience of 2+ years preferred, with at least 1 year in a home health agency, primary care clinic, or health facility.
- Valid driver's license and reliable transportation.
- Demonstrates critical thinking skills and the ability to make appropriate clinical judgments.
- Excellent written and oral communication skills.
- Proficiency in using computer systems and electronic charting.
