As an RN Case Manager, you will provide skilled nursing care while coordinating and overseeing client services in the home. You will partner with clients, families, physicians, and interdisciplinary team members to develop individualized care plans that promote health, independence, and quality of life.

• Conduct comprehensive initial and ongoing client assessments
• Develop, implement, and update individualized care plans in collaboration with physicians and the care team
• Provide direct skilled nursing care to clients of varying ages and acuity levels
• Coordinate care with clients, families, caregivers, and healthcare providers
• Perform medication management, reconciliation, and client education
• Monitor client conditions and communicate changes promptly
• Educate and support clients, families, and caregivers
• Complete timely and accurate clinical documentation using electronic charting
• Assess home environments to promote safety
• Provide clinical oversight and leadership to caregivers
• Ensure compliance with OSHA, HIPAA, and all applicable state and federal regulations
• Effectively manage your daily schedule and participate in an occasional on‑call phone rotation

• Current, unrestricted Registered Nurse (RN) license in both Minnesota and Wisconsin
• Current CPR certification
• Negative TB skin test or chest X‑ray
• Minimum of one year of nursing experience in a healthcare setting
• Home Care experience preferred
• Valid driver’s license and auto insurance
• Strong clinical assessment and critical thinking skills
• Excellent written and verbal communication skills
• Proficiency with electronic medical records and computer documentation
