Job Title:
Registered Nurse (RN) Case Manager – Home Health | A Territory You Can Truly Own
Territory: Mound, Maple Plain, Independence, Howard Lake, Delano, Montrose, Watertown, Mayer, Waverly, Wayzata, Excelsior, Cokato, Orono, Victoria, Cologne, Carver, St. Bonifacius, Minnetonka + approximately 35 miles from your home for on-call and weekend coverage
Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week
Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

- Complete OASIS assessments and individualized care plans.
- Deliver skilled nursing care and patient education.
- Collaborate with physicians and interdisciplinary team members.
- Help patients remain safe, healthy, and independent at home.
- Maintain timely and accurate clinical documentation.
- Active Minnesota Registered Nurse (RN) license.
- 3+ years of RN experience (home health experience preferred).
- Strong assessment, communication, and organizational skills.
- Ability to work independently while managing a field-based schedule.
- Reliable transportation.
