Registered Nurse (RN) Case Manager - Home Health | A Territory You Can Truly Own

Territory: Mound, Maple Plain, Independence, Howard Lake, Delano, Montrose, Watertown, Mayer, Waverly, Wayzata, Excelsior, Cokato, Orono, Victoria, Cologne, Carver, St. Bonifacius, Minnetonka + approximately 35 miles from your home for on-call and weekend coverage

Schedule: Full-Time | Monday-Friday, 8:00 AM - 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week

Compensation: $85,000 - $130,000 annually plus $6,000 in first-year bonuses

You'll become the familiar face your patients look forward to seeing while building lasting relationships throughout the communities you serve.

• Manage a home health caseload throughout your assigned territory.
• Complete OASIS assessments and individualized care plans.
• Deliver skilled nursing care and patient education.
• Collaborate with physicians and interdisciplinary team members.
• Help patients remain safe, healthy, and independent at home.
• Maintain timely and accurate clinical documentation.

• Active Minnesota Registered Nurse (RN) license.
• 3+ years of RN experience (home health experience preferred).
• Strong assessment, communication, and organizational skills.
• Ability to work independently while managing a field-based schedule.
• Reliable transportation.
