As a Palliative/Hospice Nurse Practitioner at Salus Hospice, you will work closely with a physician and interdisciplinary team to provide compassionate, patient‑centered care. You'll help ensure hospice patients meet the face‑to‑face visit requirements and will support the team in delivering exceptional care to patients and their families.
Key ResponsibilitiesConduct face‑to‑face patient visits to evaluate continued hospice eligibility in line with new regulations.
Collaborate with physicians and the interdisciplinary team to share clinical findings and adjust care plans.
Perform bedside evaluations, creating comprehensive care plans tailored to the patient’s needs.
Provide direct hospice care
, supporting both the patient and family through sensitive and crucial moments.Document all findings per CMS guidelines, ensuring accurate compliance.
Consult with supervising physicians and collaborate with healthcare providers within and outside of Salus Hospice.
2+ years of recent experience in hospice, or palliative care
.Expertise in providing care to patients with end‑stage disease
.Master’s degree in Nursing preferred.
Active Nurse Practitioner certification in California.
California state licensure to practice as a Nurse Practitioner.
Willingness to travel within a large geographic area to visit patients.
Experience in delivering high-quality, compassionate hospice care
.Deep understanding of end‑of‑life care and regulations.
Ability to follow Standardized Procedures based on guidelines established by the Board of Registered Nursing Practice Act
