Salus Hospice is urgently seeking a compassionate and skilled Hospice RN Case Manager/Admissions to join our dedicated team serving the Riverside County region. Hemet, San Jacinto, Lake Elsinore, Corona, Murrieta, and surrounding areas.

Full Time and Per Diem openings.

• Perform RN duties on hospice team.
• Serve as a RN Case Manager for the hospice Interdisciplinary Team.
• Assess for signs and symptoms of discomfort including presence of pain and take prompt and appropriate action.
• Establish a plan of care while collaborating and communicating with the Interdisciplinary Team, including physician when appropriate.
• Coordinate all hospice services provided to patients/families in all settings to ensure quality of care.

• Registered Nurse with valid CA state license.
• Minimum one year of hospice RN experience.
• Strong background in medical/surgical, ICU/CCU, or ER is helpful.
• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients' place of residence.
• Must have excellent interpersonal skills and a passion for hospice.
• Understanding of hospice philosophy and needs of the terminally ill.
• Self‑directed with ability to work with little supervision.
• Current physical and TB test results.
