Exciting Opportunity for Private Duty Home Health Nurse with Trach and Gtube Experience!

Join PACIFIC HEALTH SERVICES INC as a Private Duty Home Health LVN, where you will provide compassionate care to patients in the comfort of their homes. This role is perfect for dedicated individuals looking to make a meaningful impact in the lives of patients and their families.

Day Shift: Sunday-Thursday 6am-2pm | Friday - Saturday 6am-2pm

NOC Shift: Sunday-Thursday 10pm-6am | Friday-Saturday 10pm-6am

• Administer medications and treatments as prescribed by physicians.
• Monitor and document patient progress and vital signs.
• Collaborate with healthcare professionals to develop care plans.
• Provide education and support to families regarding patient care.
• Assist with daily living activities and mobility support.
• Ensure a safe and hygienic environment for patients.
• Maintain accurate patient records and documentation.
• Respond to emergency situations and provide first aid when necessary.

• Current California LVN license and CPR certification.
• Experience in pediatric nursing or home health care preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate demeanor and a passion for pediatric care.
• Basic computer skills for documentation and communication.
• Reliable transportation and flexibility in scheduling.
• Willingness to undergo background checks and drug screening.
