The Registered Nurse BSN embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Northwestern Medicine provides personalized, expert care for our patients through every step of their heart-health journey. CVICU Registered Nurses specialize in caring for patients in the Cardiovascular Intensive Care Unit (CVICU). These patients often present with complex cardiac conditions requiring vigilant monitoring, advanced assessments, and specialized medical interventions. CVICU nurses collaborate with a multidisciplinary care team to deliver coordinated, comprehensive care tailored to each patient’s needs.

Responsibilities
  • Performs a comprehensive assessment on all assigned patients with completed documentation.
  • Communicates changes in patient condition promptly.
  • Uses appropriate nursing diagnoses and assessment data to derive realistic outcomes and documents them as measurable goals on the plan of care; modifies the plan as indicated to meet patient goals.
  • Identifies and prioritizes patient needs and/or nursing diagnoses and documents them.
  • Collaborates with patient, family, and health care team when planning patient care; assesses discharge planning needs and makes appropriate referrals.
  • Evaluates and documents patient responses to nursing interventions and adjusts the plan of care as necessary.
  • Assesses discharge planning needs and makes appropriate referrals, including palliative care.
  • Ensures completed discharge instructions, medication reconciliation, and provides information to patient/family about the patient portal and other resources.
Patient Education/Ethics and Diversity
  • Supports patient/family/spiritual/cultural needs when assessing teaching needs; identifies learning level and readiness, and documents education records.
  • Uses teach-back methodology for medication education and new medication information.
  • Utilizes and distributes Krames for patient education and clinical condition information.
  • Incorporates culture vision as a resource in care planning and delivery.
  • Treats patients, families and staff with care, respect, and dignity; values the contributions of significant others as part of the plan of care.
  • Adheres to ethical standards of practice and requests ethics consultations when appropriate.
Evidence-Based Practice (EBP) and Research
  • Implements nursing interventions based on the established plan of care and evidenced-based care.
  • Maintains and advances the nursing profession through use of best available evidence and reputable resources.
  • Participates in clinical ladder projects or unit/organization process improvement initiatives.
Healing Safe Environment
  • Demonstrates safe medication administration with barcode verification.
  • Protects patient health, safety, and rights.
  • Delivers complete, timely, and accurate bedside handoffs.
  • Rounds hourly in collaboration with patient care technicians.
  • Maintains a safe environment of excellence and contributes to quality metrics on the unit dashboard.
  • Supports prevention of pressure ulcers, falls, CAUTIs, and central line infections as appropriate to area of practice.
  • Serves as a unit safety facilitator or resource for quality initiatives as requested.
