Registered Nurse BSN

The nurse embodies the Relationship Based Care model and performs to PPM core functions under the direction of department leadership. The nurse consistently reflects the mission, vision, and values of NM, complies with the State of Illinois Nurse Practice Act, the organization’s Code of Ethics and Corporate Compliance Program, and all relevant policies, procedures, guidelines and standards.

• Performs a comprehensive assessment on all assigned patients with documented evidence.
• Communicates changes in patient’s condition promptly.
• Uses nursing diagnosis and assessment data to derive measurable goals on the plan of care; updates the care plan as needed to address realistic patient goals.
• Identifies and prioritizes patient needs and/or nursing diagnoses and documents them.
• Collaborates with patient, family, and health care team in care planning; assesses discharge planning needs and makes appropriate referrals as necessary.
• Evaluates and documents patient responses to interventions and adjusts the plan of care accordingly.
• Assesses discharge planning needs and makes appropriate referrals, including palliative care.
• Ensures completed discharge instructions, medication reconciliation, and provides information to patient/family regarding the patient portal and other resources.

• Education and Ethics: supports patient/family needs, uses teach-back for medication education, utilizes Krames for patient education, and applies culture vision in care planning; adheres to ethical standards and seeks ethics consult when appropriate.

• Evidence-Based Practice and Research: implements interventions based on the plan of care and evidence-based sources; participates in unit/organization process improvement efforts.

• Safe Healing Environment: demonstrates safe medication administration, protects patient safety, communicates clearly at bedside hand-off, rounds with the patient care technician, and contributes to quality metrics and infection-prevention goals.

• Relationship to Colleagues and Leadership: oversees care team, demonstrates interpersonal and leadership skills, coordinates unit patient care, mentors staff, and participates in peer evaluation and collaboration across hospital services.

• Use of Technology and Unit Resources: documents in EMR, adheres to equipment guardrails, understands monitoring implications, and participates in optimization of the medical record and unit technologies.

• Self-Development: pursues internal professional development opportunities and certification; maintains accountability for professional practice and participates in professional organizations as appropriate.

• Required: Bachelor of Science in Nursing (BSN);
  Current State of Illinois Registered Professional Nurse license;
  Basic Life Support (BLS).
• Preferred: National Certification in specialty area; strong interpersonal and collaboration skills; strong analytic abilities; ADN with commitment to obtain BSN within three years of start in the clinical nurse role.
