The Registered Nurse BSN embodies the attributes of the Relationship Based Care model and performs to PPM core functions. Care-related duties under these domains include the following:

  • Performs a comprehensive assessment on all assigned patients as evidenced by completed documentation.
  • Communicates changes in patient s condition promptly.
  • Utilizes appropriate nursing diagnosis and data from the assessment to derive realistic outcomes and documents them as measurable goals on the plan of care; modifies care plan as indicated to achieve realistic patient goals.
  • Identifies and prioritizes patient needs and/or nursing diagnoses and documents them.
  • Collaborates with patient, family, and health care team when planning care; assesses discharge planning needs and makes appropriate referrals as necessary.
  • Evaluates and documents patient response to nursing interventions and adjusts the plan of care as necessary.
  • Assesses discharge planning needs and makes appropriate referrals, including palliative care; ensures completion of discharge instructions and medication reconciliation; provides information to patient/family regarding the patient portal and other resources.

Patient Education/Ethics and Diversity domain continues with:

  • Supports patient/family/spiritual/cultural needs when assessing education needs; documents education record and determines learning level and readiness.
  • Utilizes teach-back methodology for medication education.
  • Distributes Krames patient education materials; uses Culture Vision as a planning resource.
  • Treats patients, families and staff with care, respect, and dignity; values contributions of patient s significant others as part of the plan of care; adheres to ethical standards of practice; requests ethics consult when appropriate.

EBP and Research includes:

  • Implements nursing interventions aligned with the plan of care and evidenced-based practice.
  • Maintains and advances the professional image of nursing using current evidence and resources.
  • Participates in unit/organization level process improvement or LADDER projects.

Healing Safe Environment includes:

  • Safe medication administration with bar coding.
  • Protects health, safety and rights of the patient.
  • Delivers complete, timely, and accurate bedside handoffs; rounds hourly in collaboration with the patient care technician.
  • Maintains a safe environment and contributes to unit quality metrics and organizational initiatives; supports prevention of pressure ulcers, falls, CAUTI, and central line infections as applicable.
  • Participates in unit safety initiatives and huddles as requested.

Relationship to Colleagues includes:

  • Oversees and directs work of patient care or support staff; maintains accountability for patient care.
  • Demonstrates effective interpersonal leadership; handles conflicts appropriately.
  • Proficient in charge nurse/preceptor roles when assigned; coordinates patient care for the unit; leads when condition changes and consults with PCP as needed.
  • Serves as resource and role model for staff, students, and system personnel; assists with orientation and mentoring; contributes to peer evaluation and clinical case reviews; collaborates with hospital services to maintain excellence.
  • Identifies barriers to practice and communicates with nursing leadership.

Resource Utilization and Use of Technology domains cover:

  • Ensures appropriate level-of-care documentation in the EMR; understands and adheres to supply charging.
  • Participates in value-based initiatives; commits to reducing overtime utilization.
  • Ensures electronic and paper documentation completeness; uses Alaris pump guardrails; understands monitoring implications for clinical management; uses bed management system; identifies new technologies; engages in CEOC activities for medical record optimization.
