  Full time, 40 hours/week
• Hours:
  
  Monday - Friday between the hours of 7am - 6pm
• Saturday rotation (approx. once a month), 8am - 12pm
• Call rotation
• Outpatient
  - Onsite and telehealth

We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive, patient-centered care in a collaborative and supportive practice environment. This role offers the opportunity to work alongside experienced physicians and interdisciplinary team members while managing a diverse patient population across the continuum of care.

What You'll Do

• Conduct comprehensive patient assessments, examinations, and diagnoses
• Manage acute and chronic medical conditions
• Order and interpret diagnostic testing, laboratory studies, and imaging
• Develop individualized treatment plans and prescribe therapies as appropriate
• Educate patients and families on disease prevention, wellness, and treatment plans
• Collaborate with physicians and care teams to deliver exceptional patient outcomes
• Promote evidence-based practice, quality improvement, and patient safety initiatives

Qualifications:

• License:
  Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required;
  Federal DEA and Illinois Controlled Substance License required
• Education:
  
  Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience:
  Minimum of three years primary care/internal medicine APN/PA experience preferred

• Minor procedures such as skin tag removal
• Gynecological examinations such as Pap smears, breast exams, pelvic exams
• Diagnostic tests/ EKG
• POCT such as COVID/FLU/Strep
