Monday - Friday between the hours of 7am - 6pm - Saturday rotation (approx. once a month), 8am - 12pm

Job Description

We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive, patient-centered care in a collaborative and supportive practice environment. This role offers the opportunity to work alongside experienced physicians and interdisciplinary team members while managing a diverse patient population across the continuum of care.
What You'll Do
- Conduct comprehensive patient assessments, examinations, and diagnoses
- Manage acute and chronic medical conditions
- Order and interpret diagnostic testing, laboratory studies, and imaging
- Develop individualized treatment plans and prescribe therapies as appropriate
- Educate patients and families on disease prevention, wellness, and treatment plans
- Collaborate with physicians and care teams to deliver exceptional patient outcomes
- Promote evidence-based practice, quality improvement, and patient safety initiatives
Qualifications:
- License:
Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required;
Federal DEA and Illinois Controlled Substance License required - Education:
Completion of master’s level program in specialty area - Certification: BLS/CPR certification required
- Experience:
Minimum of three years primary care/internal medicine APN/PA experience preferred
Special Knowledge and
Skills Required:
- Minor procedures such as skin tag removal
- Gynecological examinations such as Pap smears, breast exams, pelvic exams
- Diagnostic tests/ EKG
- POCT such as COVID/FLU/Strep
