Salary

The hourly rate for this position ranges from $61.00 to $79.67, depending on relevant work experience and other factors. Placement within the range follows internal equity guidelines and, where applicable, the collective bargaining agreement.

Description

Northwestern Medicine has an exciting opportunity for a Physician Assistant or Acute Care Nurse Practitioner to join our Hospital at Home team. The position is inpatient and scheduled for 30 hours per week, three days from 7:00 am to 5:00 pm. Office call is required.

Essential Functions
  • Demonstrate clinical expertise and expanded knowledge through patient care activities, including diagnosis and management of a wide range of health problems.
  • Assume responsibility for the health outcome of an assigned patient population.
  • Consult with collaborating physicians and other health professionals in areas of clinical expertise.
  • Develop educational systems for patients, families, and staff.
  • Mentor graduate students in advanced practice and patient care.
  • Actively participate in quality improvement and/or research relevant to your area of expertise.
Responsibilities
  • Conduct comprehensive assessments of patients' health status through histories and physical examinations.
  • Provide direct clinical care to patients.
  • Prescribe/orders medications, therapies; order/interpreter laboratory tests, x‑rays, and procedures per clinical privileges.
  • Evaluate patients to determine the need for further health care.
  • Deliver comprehensive teaching to patients and families.
  • Document all care and education in the medical record.
  • Refer to other disciplines as appropriate.
  • Act as a consultant to other disciplines in specialized areas.
  • Integrate research‑based knowledge and clinical expertise in decision‑making.
  • Maintain a professional image and advocate for patients, families, coworkers, and the community.
  • Keep certifications and continuing education current for licensure and privileging.
  • Serve as a patient advocate and model “Patients First.”
  • Participate in relevant committees, task forces, and quality improvement initiatives.
  • Provide teaching strategies for patients, families, and staff.
  • Develop and participate in continuing education programs for staff.
  • Precept students and deliver formal education to internal and external groups.
ADDENDUM

The APP serving in the Hospital at Home Admitting role is responsible for clinical evaluation, medical eligibility determination, and ensuring a safe, patient‑centered transition from the hospital (or ED) into the home setting. The role collaborates closely with hospitalists, consultants, case management, nursing, and the Hospital at Home team to drive efficient patient throughput while maintaining high standards of clinical quality and patient experience.

This high‑impact role is ideal for clinicians who enjoy autonomy, rapid clinical decision‑making, systems‑based practice, and care model innovation.

Core Responsibilities
  • Clinical Evaluation & Admission/Transfer: assess eligibility for Hospital at Home and perform comprehensive medical screening.
  • Assess clinical stability, acuity, and appropriateness for inpatient‑level care in the home.
  • Perform history and physical exams with complete documentation.
  • Enter admission orders and develop initial plans of care.
  • Reconcile medications and coordinate durable medical equipment, diagnostics, therapies, and specialty consults prior to transfer.
  • Patient Acquisition & Throughput: review eligibility lists, identify candidates, and collaborate with interdisciplinary teams to facilitate appropriate referrals.
