Registered Nurse (RN) Home Health – Lake Forest, IL.

Full‑time, 40 hours per week. 8‑hour shifts. Ongoing employment. Paid per visit with a 4‑month orientation pay guarantee, a $10,000 first‑year success bonus, monthly clinical outcome bonuses, and other financial incentives.

• Lead and collaborate to provide management and delivery of patient care plans in a team‑based environment.
• Deliver quality patient care during visits, spending approximately 20% of the day traveling to patient locations.
• Accurately complete charting and meet critical deadlines.
• Self‑manage time and schedule for patient visits.
• Use tablets, mobile phones, and EMR software to document care.
• Participate in the 24/7 after‑hours care team as needed.

• RN license in the state where you will work.
