Nurse Navigator-Medical Oncology

Job Overview

Full‑time (40 hours per week) Nurse Navigator position located at Lake Forest Hospital. Supports the Hem Onc Inpatient team with care coordination on the unit and edule:
Monday–Friday, 8:00 am–4:30 pm.

• Coordinate and plan physician‑requested multidisciplinary conferences; track scheduled procedures and follow pathology results.
• Interpret and guide patients through follow‑up for laboratory, radiology and pathology procedures.
• Follow up on patients with complicated or specialized needs.
• Work with patients during the rehabilitation and recovery period following a cancer diagnosis.
• Serve as a resource for patients and families while coordinating additional resources as requested.
• Coordinate patient referrals to insurance companies and doctors’ offices.
• Monitor quality of care and physician/patient satisfaction through follow‑up.
• Teach patients about cancer health concerns and provide pertinent lectures as needed.
• Educate patients on expectations prior to, during, and after a cancer diagnosis and treatment procedures.
• Evaluate patients’ physical and emotional recovery after treatment, utilizing appropriate resources as needed.
• Develop and implement survivorship care plans for patients completing treatment.
• Identify, recommend, and establish supportive services to meet patients’ needs.
• Develop programs and provide staff and community education as required; maintain reference library.
• Pursue opportunities for public speaking to promote cancer care services at Northwestern Medicine Lake Forest Hospital.

Required:

• Registered Nursing license issued by the State of Illinois.
• Bachelor of Science in Nursing.
• BLS certification through the American Heart Association.
• Demonstrated broad knowledge of oncology care theories, principles, and practices.
• Five years of oncology‑related work experience.
• Self‑directed independent practitioner.

Preferred:

• Master of Science in Nursing.
