Overview

Geneva Lake Manor has a unique opportunity for a long-term care part-time Social Service Assistant.

Job Summary: As the Social Service Assistant at Geneva Lake Manor, you will work under the direction of the licensed Social Services Director and be responsible for assisting in the promotion of the general emotional and psychosocial needs of residents. You will assist acting as the liaison among departments within Geneva Lake Manor as well as assist the discharge planner working with community resources to ensure resident needs are met after they are discharged.

You will assist in completing resident interviews and assessments, assist in coordinating resident care plan meetings and assist in developing and executing resident care plans based upon resident needs following state and federal regulations.

Skills and Qualifications
  • Must be a high school graduate
  • Must have a valid driver’s license
  • Must be organized and self-motivated
  • Must have ability to work independently
  • Experience working with the elderly population in a professional setting
  • Must be able to communicate effectively and professionally with staff and families
  • Ability to plan, organize and implement individual resident care plans
  • Ability to work on the computer related to the MDS and other programs as needed
  • Ability to carry out public relations activities
  • Must have the ability to multi-task.
  • Must have the ability to manage stressful situations
  • Must be able to meet deadlines
  • Must be able to maintain confidentiality
  • Must be able to stand or walk 4-6 hours in an 8 hour day
  • Must be able to occasionally bend, squat, and twist body during an 8 hour day.
  • Must be able to life a maximum weight of 20# with frequent lifting, carrying, pushing and pulling of objects up to 10#
Job Responsibilities
  • To attend resident care planning sessions and develop realistic approaches and goals for the resident.
  • To document residents response to the care plan in the medical record as required by state and federal guidelines.
  • To document quarterly notes in the medical record and other pertinent information as needed.
  • To act as resident advocate in the delivery of care
  • To communicate with resident’s on social and emotional needs
  • To work under the direction of the Social Services Director for matters concerning admission and discharges; i.e., assist with on-site visits to the hospital or other referral locations; complete necessary documentation/forms; orient new residents to the facility; taking phone calls related admission/discharge; etc.
  • To participate in committees as assigned by the department director.
  • Any and all other duties as assigned by Administrator in an effort to maintain a quality social services department.
  • Provides services to meet medically related social and emotional needs of the resident
