Job SummaryThis position will provide and administer occupational physical therapy to patients and residents in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. We aim to deliver excellent services to guests who have chosen Geneva Lake Manor for rehabilitation and nursing care.
Compensation- $40.00 - $50.00 per hour, dependent on experience

Key Responsibilities- Uphold the professional, legal, and ethical standards of occupational therapy as defined by the AOTA and State Practice Act.
- Ensure service and operational excellence of all assigned activities, meeting or exceeding expectations of patients and stakeholders.
- Perform initial patient examination, evaluation, and interpretation of referrals, and create the initial plan of care for each patient, revising as appropriate.
- Create and maintain a patient record for each treatment.
- Perform other duties as required and/or assigned.
- Graduate of an accredited physical therapy education program.
- CPR certification preferred.
- Licensed in the state of practice.
- Not excluded or sanctioned from state or federal programs.
- Demonstrate a professional attitude and appearance.
- Proficient in administrative and communication tasks within the department, facility, and company.
- Possess thorough knowledge of the principles, methods, materials, and equipment used in the practice of the discipline.
- Display working knowledge of multidisciplinary practice scopes in Outpatient, ALF, ILF, or LTC settings.
- Have thorough knowledge of payer source documentation and payment requirements.
- Use analytical skills to develop outcome‑driven solutions.
- Hiring manager contact within 1–2 business days.
- 1–2 interviews with leadership and mentor.
- Electronic onboarding and paid orientation.
