- Maintain accurate records and comply with safety protocols
- DDS/DMD degree from a U.S. accredited dental school or AEGD/GPR residency program
- Active Texas dental license in good standing
- Active Federal DEA and applicable certifications
- 3-5 plus years clinical experience preferred
