Physical Therapist; PT

Seeking Physical Therapists in the Home Health setting to cover the Lake Jackson area.

• $75 per regular PT Visit
• $112.50 per PT Eval
• $187.50 per PT SOC

Physical Therapist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.

• Evaluate patients and perform appropriate therapy interventions.
• Develop a comprehensive plan of care and coordinate multidisciplinary care.
• Perform prompt clinical documentation and visit notes.
• Maintain continued communication with physician, care team members, patient and family.
• Comfortable working in all aspects of the home health setting.
• Implement and maintain physical therapy protocols.
• Manage patient treatment and outcomes directly, or through PTA supervision.

• Current Texas Physical Therapist (PT) license.
• One year of physical therapy experience.
• Experience with Homecare Homebase is helpful, but not required.
• Previous Home Health experience is helpful, but not required.
• Current Texas driver's license.
• CPR certification.
