Job Summary and Responsibilities
As our Social Worker, you will be a compassionate advocate, providing vital support and guidance to individuals and families facing complex challenges. Every day, you will conduct psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect clients to resources. You'll advocate for clients and collaborate with multidisciplinary teams to ensure holistic support and promote well‑being. To be successful in this role, you will possess exceptional interpersonal skills, empathy, strong knowledge of community resources, and proven crisis management abilities, fostering positive change and client empowerment.
- Conduct and document psychosocial assessment of patients
- Share responsibility in identifying appropriate decision makers if patient is without capacity, educating all in patients’ rights and encouraging participation in decision‑making and care
- Provide liaison services and referrals to hospital resources and community agencies, including medication assistance programs
- Identify mental health needs of patients and provide counseling and referral as indicated
- Collaborate with interdisciplinary healthcare team members to ensure continuity and coordination of patient care throughout the continuum and into the community by coordinating placement with home health agencies, long‑term acute hospitals (LTACs), skilled nursing facilities (SNFs) and assisted living facilities, nursing home placement, acute rehab, psych placement, and custodial care facilitation
- Serve as a resource to the patient, family, and healthcare team regarding emotional and behavioral issues and refer to community services available to adolescents, adults and elders
- Master’s degree from a Council on Social Work Education accredited graduate school of social work and three (3) years clinical social work experience upon hire
- Three (3) years allied health practice, with hospital or medical background or experience
