- Lead and mentor the care staff in delivering person‑centered care.
- Maintain compliance with state and federal regulations and uphold Mary's Woods' quality assurance standards.
- Collaborate with interdisciplinary teams to ensure the best care for residents.
- Participate in care conferences and stay informed about residents' medical diagnoses and conditions.
- Foster positive relationships with employees, residents, visitors and regulatory bodies.
- Responsible for resident assessments, including admission, change of condition, and quarterly assessments.
- Communicate on an ongoing basis with employees regarding performance expectations.
- Promote the growth of staff through coaching and formal education in areas such as teamwork, decision‑making, clinical skills, quality and safety.
- Be visible and available to all shifts and employees through rounding, staff meetings and other methodologies of employee connection and engagement.
- Conduct investigations of incidents, follow up on incident reports and conduct root cause analysis if indicated.
- Monitors trends from incidents and incorporates into quality improvement activities.
- Unencumbered RN or LPN license in the State of Oregon.
- Minimum of one year experience in an assisted‑living or community care setting preferred.
- Minimum of two years of management experience.
- Current CPR and First Aid certification within three months of hire.
