Role Description

The Registered Nurse at Age Right Care at Home is a full-time, on-site role based in Lake Oswego, OR. This role is responsible for assessing client health needs, developing and updating individualized care plans, and coordinating services to support clients living safely
-to-day tasks include administering medications and treatments, monitoring vital signs and symptoms, documenting care, and communicating changes in condition to clients, families, and care teams. The RN will provide education on health conditions, medications, and self-care, and collaborate with caregivers and other healthcare professionals to promote safety, comfort, and continuity of care. The role also involves participating in quality improvement initiatives and ensuring adherence to clinical standards and regulatory requirements.

Qualifications
  • Active Registered Nurse (RN) license in the state of Oregon and graduation from an accredited nursing program.
  • Clinical care skills including patient assessment, medication administration, care planning, and monitoring of chronic and acute conditions.
  • Strong communication and interpersonal skills to collaborate with clients, families, caregivers, and interdisciplinary care teams.
  • Organizational and time-management abilities to prioritize visits, manage documentation, and coordinate services in a home-based setting.
  • Knowledge of home care best practices, safety protocols, and relevant regulations; prior home health or geriatric experience is preferred.
  • Ability to work independently, exercise sound clinical judgment, and adapt to changing client needs.
  • Basic proficiency with electronic health records and related clinical documentation tools.
  • Current CPR certification and ability to meet Age Right Care at Home pre-employment requirements.
