Registered Nurse – Home Health

Schedule:

Assignment Details:

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software experience
• OASIS experience
• CHPN certification (preferred)
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance
• 1-2 years of experience as a Registered Nurse in a clinical care setting
• 2+ years of home health experience

Roles & Responsibilities:

• Assess home health and/or hospice patients to identify their physical and psychosocial needs.
• Implement and evaluate the Plan of Care (POC) to ensure quality care.
• Educate patients and families on care, medication, and safety practices.
• Supervise and provide guidance to nursing assistants and other staff.
• Monitor cases for compliance with third-party payer requirements.
• Provide effective communication among patients, families, and interdisciplinary team members.
