Cardiac Rehabilitation Coordinator

Provides direct patient care to cardiac and pulmonary rehab patients including various types of monitoring, education and therapeutic exercise programs.

• Assesses patient needs daily through chart review, unit nurses or cardiac nurse, physicians and interdisciplinary team.
• Provides exercise prescription for patients utilizing test data, patient status and current medication regimen; updates prescription according to the patient's response.
• Schedules and coordinates patient assessment, testing, results reporting, treatment and follow up as needed.
• Assesses hemodynamic response and patient tolerance to activity progression and utilizes information to provide appropriate exercise prescription.
• Performs appropriate documentation of monitored ambulation, education and discharge planning per departmental policies, guidelines and practices as pertinent for setting.
• Participates in routine maintenance and calibration of the test equipment to ensure its proper operating condition and reporting the need for major repair.
• Assists in recruiting/interviewing and onboarding of new associates.
• Creates quality improvement monitoring strategies and improvement initiatives.
• Assists in monitoring of staff performance.
• Provides exercise information for patients, peers and the community which may include public speaking and education classes.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.

Bachelor's degree

No experience required

• Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
• Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
• Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
• Frequent keyboard use/data entry.
• Occasional bending, stooping, kneeling, squatting, twisting and gripping.
• Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
• Rare climbing.
