Description:

Home Health Physical Therapist responsible for home visits.
Periodic office meetings required.
Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
Average number of patients per day: 5-6.
Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points.
Types of cases include Start of Care and Follow Ups.
Must be willing to travel up to 75 miles from the home clinic.
Mileage reimbursed at the IRS standard rate.
Commute to and from the first and last patient are not billable.
Weekend requirements vary per location and profession.
Candidates will be compensated for driving from patient home to patient home.
This position may involve a weekend rotation with 1 Saturday per month for admission needs.
No on-call requirements for this position.
Expected to accept all patients on the schedule regardless of their distance from prior appointments.
All documentation must be completed within 24 hours of the appointment with the patient.
Delayed or incomplete documentation may impact billing and follow-up patient care.
There are no guaranteed hours for this contract.
Clinicians must achieve weekly productivity points of 30, which equates to 40 hours per week.
All hours worked must be pre-approved.
Travelers must be proficient in the required EMR systems.
Candidates must confirm they are aware this is a home health assignment.
Candidates must confirm they have a vehicle and insurance for driving to patient homes.
Candidates should be comfortable with longer drives between patients.
Local candidates are accepted.
