Job Overview

Graham Healthcare Group is seeking a Registered Nurse (RN) for Home Health in Lake Zurich, Illinois. This full‑time position supports patients in Palatine, Huntley, Crystal Lake, and surrounding areas.

• Lead and collaborate to manage and deliver patient care plans.
• Provide nursing care at patient homes and ensure continuity of care.
• Maintain accurate and timely charting to meet critical documentation deadlines.
• Self‑manage time and schedule to optimize patient visits.
• Utilize tablets, mobile phones, and EMR software for clinical documentation.
• Travel approximate 20% of the day to and from patient locations.

• Current Registered Nurse license in the state where you will work.
• Graduate of an approved school of practical nursing.
• Minimum one year of RN experience with the clinical competence to deliver quality patient care.
• Current driver’s license and ability to drive to patient locations.
• Commitment to consistently meet critical charting deadlines.
• Experience with tablets, mobile phones, and EMR software.
