Transfer Coordinator

Peace River Center seeks a Transfer Coordinator to facilitate safe, timely transitions of behavioral health clients across inpatient, crisis, and community programs. The coordinator gathers clinical and administrative information, verifies eligibility, secures authorizations, and schedules placements while maintaining accurate electronic health records. This role collaborates closely with clinicians, case managers, hospitals, and community partners to ensure continuity of care and adherence to regulatory, payer, and organizational guidelines.

The Transfer Coordinator also provides clear communication to clients and families, supports problem solving for complex cases, and upholds our mission-driven, client-centered values.
