Job Title: Physical Therapist
Facility: Breezy Hills Rehabilitation and Care Center
Location: Lakeland, FL
Shift: Full-Time
Salary: $48-$53 per hour

• Evaluate residents and establish appropriate treatment goals and methods.
• Implement physical therapy treatment plans and maintain thorough documentation.
• Apply therapeutic techniques in gait, ambulation, muscle strength, coordination, endurance, posture, balance, and sensory awareness.
• Design and fit adaptive devices, including orthotics and prosthetics, and instruct residents in their use.
• Select physical modalities tailored to each resident's needs.
• Collaborate with the rehabilitation team to ensure effective therapy regimens.
• Monitor and assess residents’ progress in mobility and functional activities.
• Adapt programs to meet individual resident needs.
• Participate in resident care and staffing conferences.
• Supervise supportive personnel following state and professional guidelines.
• Conduct training programs for medical, nursing, and other staff on physical therapy techniques.
• Order, maintain, and report equipment needs for repair.
• Foster a caring and compassionate environment for residents and their families.

• Current, active, and unencumbered Florida Physical Therapist license.
• Minimum of 1 year of experience in a nursing home preferred.
