Position Summary

The Transitions of Care (TOC) clinical pharmacist works within the scope of pharmacy laws and standards of practice to maximize benefits of drug therapy while identifying opportunities to improve quality, safety, and costs associated with medication use across care transitions. The TOC clinical pharmacist supports the mission of the hospital by aiming to improve medication reconciliation across the continuum of care and to reduce patient readmissions.

He/she accomplishes this by performing clinical duties in collaboration with the multidisciplinary team, engaging in direct patient communication regarding medication use, as well as ensuring post-discharge follow up and continuity of care.

    • Standard Work Duties:
      Transitions of Care Clinical Pharmacist
  • Actively participates in pharmacy team development and accomplishing departmental goals, objectives, and priorities.
  • Provides comprehensive pharmaceutical care for patients in a manner consistent with consensus guidelines and evidence-based medicine. Evaluates drug therapy for appropriate indications, dosing, drug interactions, drug allergies, and effectiveness. Intervenes to improve medication safety and prevent adverse drug reactions, treatment gaps, duplicate therapies, adherence and cost issues, as well as drug-drug and drug-disease interactions.
  • Ensures accuracy in admission and discharge medication reconciliation. Provides education to patients regarding medications and disease states during hospitalization and/or after discharge.
  • Actively participates as a member of the multidisciplinary team to provide cost-effective and efficient medication and resource utilization. Collaborates with clinicians and support staff to ensure continuity of care and patient safety with a goal of reducing patient readmissions.
  • Participates in pharmacotherapy research and/or quality improvement efforts with a focus on improvement in patient outcomes. Coordinates the development, implementation, education, and assessment of evidence-based pharmacotherapy protocols.
  • Actively participates on hospital committee(s) related to area of expertise, when applicable. Provides consultation to hospital committees, such as Pharmacy and Therapeutics (P&T), when pharmacotherapy issues related to area of expertise are discussed. Assists with monograph development or formulary reviews when needed.
  • Provides education to pharmacists, providers, nurses, and other healthcare team members regarding drug-related policies and procedures as well as other pharmacotherapy topics as necessary. Participates in the education of pharmacy residents and students, and overall residency program requirements and activities as outlined in the LRH Pharmacy Residency Program Manual and in accordance with the ASHP Accreditation Standard.
