Position Summary

Under general direction, the Medical Assistant works with patients, families, providers, other providers, and staff to promote timely access to needed care, providing daily continuity of care coordination, and coaching patients, families, and caregivers to understand the patient's care plan and self‑care management responsibilities. Responsibilities, knowledge, skills, abilities, and working conditions may change as needs evolve. Performs other related duties as assigned.

    • Actively participates in team development, achieving dashboards, and in accomplishing department goals and objectives.
    • Assist in delivering quality care to providers, patients, patients' families, and caregivers. Be aware and sensitive to cultural differences between patients and co‑workers. Demonstrate personal responsibility and respect for patients, their families, and co‑workers in professional appearance.
    • Perform job related duties such as vital signs, phlebotomy, assisting with procedures as trained, and EKG. Engage patients, patients' families, and their caregivers in understanding and monitoring patient self‑management care plans in a manner that is culturally and linguistically appropriate to the patient and caregiver.
    • Collect and document data related to patient needs.
    • Develop and maintain relationships among patients and their families, care team that supports their access to the medical home. Provide timely communication with patients, make inquiries, and execute follow‑up actions between disciplines.
    • Participate in continuing professional growth through attendance at workshops and professional in‑services and through independent research and reading to include timely and respectful communication skills.
    • Demonstrates a complete understanding of their assigned specialty's service line practices and standards. Follows departmental standard operating procedures (SOP) and policies.
