Position Summary

Provides pharmaceutical care to critically ill or ED patients according to expert judgment and LRH policies and procedures. Work within the scope of Pharmacy Laws and Standards of Practice to maximize benefit to the patient while minimizing associated costs. Functions include rounding as a member of the multidisciplinary critical care team, if applicable, and to provide pharmacotherapeutic management for all ICU/ED patients, responding to resuscitation events, and coordinating the development and implementation of drug therapy protocols to maximize benefits of drug therapy.

He/she must identify opportunities for and contribute to the improvement of quality, safety, and costs, as well as patient, customer, and employee satisfaction.

Standard Work Duties:
Critical Care Clinical Pharmacy Specialist
  • Actively participates in pharmacy team development and accomplishing department goals and objectives. Remains current with all LRH mandatory annual education.
  • Provides comprehensive pharmaceutical care (pharmacokinetic monitoring, nutritional support, patient‑specific dosing of medications with consideration of age, renal and hepatic insufficiency, underlying conditions, provision of drug information, etc.) for the critically ill/ED patient in a manner consistent with consensus guidelines and evidence‑based medicine.
  • Evaluates drug therapy for appropriate indications, dosing, drug interactions, and drug allergies. Monitors the patient's pharmacotherapeutic regimen for effectiveness and adverse drug events/reactions and intervenes as needed. Reports adverse drug events/reactions in appropriate institutional system(s) and assists in determining root cause(s) and any potential improvements in the medication use system.
