Position SummaryUnder general direction, the Medical Assistant works with patients, families, providers, other providers, and staff to promote timely access to needed care, providing daily continuity of care coordination, and coaching patients, families, and caregivers to understand the patient’s care plan and self‑care management responsibilities. Responsibilities, knowledge, skills, abilities, and working conditions may change as needs evolve. Performs other related duties as assigned.

- Actively participates in team development, achieving dashboards, and in accomplishing department goals and objectives.
- Assists in delivering quality care to providers, patients, patients' families, and caregivers. Be aware and sensitive to cultural differences between patients and co‑workers. Demonstrate personal responsibility and respect for patients, their families, and co‑workers in professional appearance.
- Performs job‑related duties such as vital signs, phlebotomy, assisting with procedures as trained, and EKG. Engages patients, patients' families, and their caregivers in understanding and monitoring patient self‑management care plans in a manner that is culturally and linguistically appropriate.
- Collects and documents data related to patient needs.
- Develops and maintains relationships among patients and their families, care team that supports their access to the medical home. Provides timely communication with patients, makes inquiries, and executes follow‑up actions between disciplines.
- Participates in continuing professional growth through attendance at workshops and professional in‑services and through independent research and reading to include timely and respectful communication skills.
- Demonstrates a complete understanding of their assigned specialty's service line practices and standards. Follows departmental standard operating procedures (SOP) and policies.
- Administers prescribed medications and treatments in accordance with provider directives.
